<p>The <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml12/12181.html?tab=recalls">U.S. Consumer Product Safety Commission</a>, in cooperation with the Aqua-Leisure Industries, has announced a voluntary recall of the following consumer product.</p>
<p><strong>Name of Product: </strong>First Fitness® Trampolines with Handlebars</p>
<p><strong>Units: </strong>About 40,000</p>
<p><strong>Manufacturer: </strong>Aqua-Leisure Industries Inc., of Avon, Mass.</p>
<p><strong>Hazard: </strong>Metal fatigue can cause the handlebar to break away during use, posing a risk of laceration from exposed metal surfaces or other injury from a fall.</p>
<p><strong>Incidents/Injuries: </strong>Aqua-Leisure has received four reports of handlebars breaking from the metal connection joint during use. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall involves First Fitness Kid’s First trampolines with handlebars. The child-size toy trampolines have a red and blue metal handlebar, a blue nylon deck guard and a black jumping deck. “First Fitness” is embossed on the jumping deck in white letters. The trampolines can be identified by model number FF-6902TR and Toys R Us SKN 491463.</p>
<p>The model and store numbers can be found on the lower right corner of the back of the packaging. A sewn-in tag on the bottom of the deck lists the factory date code of five numbers followed by “GLTX.”</p>
<p><strong>Sold Exclusively at: </strong>Toys “R” Us stores nationwide from September 2010 through April 2012 for between $45 and $70.</p>
<p><strong>Manufactured in: </strong>China</p>
<p><strong>Remedy: </strong>Consumers should immediately take the recalled trampolines from children and contact Aqua-Leisure’s recall hotline for a full refund.</p>

<p>Tots in Mind Inc, a company that is no longer in business, had recalled crib tents and play yards in 2010.  Consumers can identify their tent by the 2&#8243;x 1½&#8221; label with Tots In Mind logo located on the non-mesh portion near the top of the tent.</p>
<p><strong>Products: </strong>Portable Playard Tent</p>

<p><strong>Units:</strong> 330,000</p>
<p><strong>Incidents/Injuries:</strong>  In 2008, <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml10/10303.html">a two year old boy died</a> after becoming entrapped between the bottom rail of a play yard tent and the top rail of a play yard. The fatality was reported in a prior recall with the firm in July 15, 2010.</p>
<p>In 2007, a two year old boy sustained a catastrophic brain injury when his crib tent inverted and the product&#8217;s broken rod trapped him at the neck. The remaining 25 reports to CPSC involved children being trapped between the tent and the crib/play yard.  There were also failures regarding the tents&#8217; fabrics and or zippers.  Three of these 25 reports also resulted in injuries.  In one of these cases, a parent found her child turning blue while stuck between the product and the top rail of the play yard.</p>
<p><strong>Description: </strong>No model names or numbers are located on the tents.  However, they can be identified by the Tots in Mind logo on top of the tent and by viewing photos at the <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml12/12179.html">CPSC website</a>.</p>
<p>The recalled products were sold at numerous retail stores for between $60 and $85 dollars.</p>

<p><a href="http://www.djgusa.com/usa/eng/Safety-Notices/Detail/529-Voluntary-Recall">Dorel Juvenile Group</a> (DJG) and the National Highway Traffic Safety Administration (NHTSA) have issued a voluntary recall involving certain Safety 1st rear-facing infant child restraints.  DJG&#8217;s data shows that some consumers purchased an infant restraint unit that did not include a base with lower anchors and tethers for children, otherwise known as LATCH.  The number of units being recalled are 1,737.</p>
<p>The affected models are: Comfy Carry: IC123FSM and OnBoard 35: IC124FSM.</p>
<p>Dorel’s products are sold at these national chains: Meijer, Bed Bath &amp; Beyond, Walmart, and Babies “R” Us, and through online retailers, including Amazon.com, BabyAge.com, and Diapers.com.</p>
<p>In the press release, Dorel also advised that customers should not return the affected products back to the stores; instead, contact the manufacturer.</p>
<p>Customers can call Dorel at (877) 416-8111 Monday through Friday from 8 a.m. to 5 p.m., or contact the company at infantlatchrecall@djgusa.com.</p>
<p>They will provide you  information on obtaining the free infant base with LATCH.</p>

<p>The U.S. Consumer Product Safety Commission have announced voluntary recall of the Kolcraft Enterprise product that is listed below.   Consumers should stop using the product immediately.  It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p><strong>Name of Product:</strong> Kolcraft Tender Vibes and Light Vibes bassinets</p>
<p><strong>Units:</strong> About 46,000</p>
<p><strong>Incidents/Injuries:</strong> CPSC and Kolcraft have received seven reports of latches that detached from the bassinet frame. One infant received a bruised cheek when the bassinet detached from the metal frame and landed sideways on the floor with the infant inside.</p>
<p><strong>Description:</strong><strong> </strong>This recall includes Kolcraft Tender Vibes bassinets with model numbers KB021-ARC, KB022-VER, KB039-CMR1 and Light Vibes bassinet with model number KB043-BNT1. A label with the model/item number of the bassinet is located on one of the legs of the metal frame. The recalled bassinets were manufactured from July 2008 through May 2010. The words “Kolcraft Tender Vibes” or “Kolcraft Light Vibes” is located on the removable music box which is attached to the side of the bassinet.  The latches that attach the bassinet base onto the metal frame appear to be locked in place but are actually still unlocked. The bassinet will then become detached from the frame, causing the entire bassinet to fall.  This has resulted in infant injury.</p>
<p><strong>Sold at:</strong> Mass market and independent juvenile specialty stores nationwide and online from July 2008 through May 2012 for between about $50 and $100.</p>
<p><strong>Manufactured in:</strong> China</p>
<p><strong>Remedy:</strong> Consumers should immediately stop using the recalled bassinets and contact the firm by phone or on-line to receive a repair kit and instructions for securing the latches to the metal frame. In the meantime, parents are urged to find an alternate, safe sleeping environment for the child, such as a crib that meets current safety standards or play yard depending on the child’s age.</p>

<p><strong>Name of Product:</strong> Safety 1st Cabinet Locks Recalled </p>
<p><strong>Units:</strong> About 900,000</p>
<p><strong>Incidents/Injuries:</strong> DJG has received 200 reports of locks that did not adequately secure the cabinet, including reports of damaged locks. Of the reported incidents, the firm is aware of 140 children between the ages of 9 months and 5 years who were able to disengage the locks and gain access to the cabinet&#8217;s contents. In three of the reported incidents, the children who gained access swallowed or handled dishwashing detergent, window cleaner or oven cleaner, and were treated, observed and released from emergency treatment centers.</p>
<p><strong>Description: </strong>This recall involves Safety 1st Push &#8216;N Snap cabinet locks with model numbers 48391 and 48442. The model numbers are printed on the back of the product and on packaging. The locks are used to secure cabinets with two straps that wrap around the knobs or handles on a cabinet door. When the product is in the lockposition, a green triangle is shown through a window on the device. The Safety 1st logo is embossed on the front of the lock. Locks manufactured between January 2004 and November 2010 are included in the recall. The date of manufacture is embossed on the back. The arrow on the date wheel points to the month and the numbers of either side of arrow represent the year of manufacture.</p>
<p><strong>Sold at:</strong> Bed Bath &#038; Beyond, and other retail stores nationwide and online at Amazon.com from January 2004 through February 2012 for between $2 and $4.</p>
<p>Manufactured in: China</p>
<p>Remedy: Consumers should immediately remove the recalled locks from cabinets and contact DJG for a free replacement Push &#8216;N Snap lock with model numbers HS158 or HS159. When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children.</p>

<p><b>Name of Product: </b>Rockland Furniture Drop-side Cribs</p>
<p><b>Units: </b>About 16,700</p>
<p><b>Manufacturer: </b>Nan Far Woodworking Co. Ltd., of Taiwan</p>
<p><b>Importer: </b>J.C. Penney Purchasing Corp., of Plano, Texas</p>
<p><b>Hazard: </b>The cribs&#8217; drop sides can malfunction, detach or otherwise fail, causing part of the drop side to fall out of position, creating a space into which an infant or toddler can roll and become wedged or entrapped, which can lead to strangulation or suffocation. A child can also fall out of the crib. Drop-side incidents can also occur due to incorrect assembly and with age-related wear and tear.</p>
<p><b>Incidents/Injuries: </b>CPSC and the firms are aware of five incidents involving drop sides that malfunctioned or detached, including one report of a child who became entrapped and sustained minor scratches and bruises.</p>
<p><b>Description: </b>This recall includes Rockland Furniture brand drop-side cribs imported and sold by jcpenney from 2005 to 2008. The wooden cribs used plastic hardware to attach the drop side. &#8220;Rockland Furniture&#8221; and the model number can be found on a rectangular label located on the lower portion of the headboard panel. The following six drop-side crib models are included in this recall:</p>

<p><b>Important Message from CPSC: </b>CPSC reminds parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for parts separating that can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers. Crib age is a factor in safety. At a minimum, CPSC staff recommends that you do not use a crib that is older than 10 years old. Effective June 28, 2011, new, mandatory federal crib rules require that all cribs manufactured and sold after that date must meet <a href="http://www.cpsc.gov/nsn/cribrules.pdf">new and improved safety requirements</a> (pdf). <p>The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p><b>Name of product: </b>Push ‘N Snap Cabinet Locks</p>
<p><b>Units: </b>About 900,000</p>
<p><b>Importer: </b>Dorel Juvenile Group (DJG) Inc., of Columbus, Ind.</p>
<p><b>Hazard: </b>Young children can disengage the cabinet locks, allowing access to cabinet contents and posing the risk of injury, due to dangerous or unsafe items.</p>
<p><b>Incidents/Injuries: </b>DJG has received 200 reports of locks that did not adequately secure the cabinet, including reports of damaged locks. Of the reported incidents, the firm is aware of 140 children between the ages of 9 months and 5 years who were able to disengage the locks and gain access to the cabinet’s contents. In three of the reported incidents, the children who gained access swallowed or handled dishwashing detergent, window cleaner or oven cleaner, and were treated, observed and released from emergency treatment centers.</p>
<p><b>Description: </b>This recall involves Safety 1st Push ‘N Snap cabinet locks with model numbers 48391 and 48442. The model numbers are printed on the back of the product and on packaging. The locks are used to secure cabinets with two straps that wrap around the knobs or handles on a cabinet door. When the product is in the “lock” position, a green triangle is shown through a window on the device. The Safety 1st logo is embossed on the front of the lock. Locks manufactured between January 2004 and November 2010 are included in the recall. The date of manufacture is embossed on the back. The arrow on the date wheel points to the month and the numbers of either side of arrow represent the year of manufacture.</p>
<p><b>Sold at: </b>Bed Bath &#038; Beyond, and other retail stores nationwide and online at Amazon.com from January 2004 through February 2012 for between $2 and $4.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately remove the recalled locks from cabinets and contact DJG for a free replacement Push ‘N Snap lock with model numbers HS158 or HS159. When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children.</p>

<p>Johnson &#038; Johnson is recalling all 574,000 bottles of grape-flavored, liquid Infants&#8217; Tylenol from stores nationwide due to problems with recently redesigned bottles. </p>
<p>17 parents or caregivers have complained that a protective cover on the top of the bottles didn&#8217;t work correctly. The protective cover is meant to limit how much of the liquid can be drawn into the syringe; instead, when consumers insert the plastic syringe it pushes the protective cover into the bottle. </p>
<p>J&#038;J says there have been no reports of babies being harmed with excess doses.</p>
<p>The bottle was redesigned 3 months ago in an attempt to provide parents with a safer bottle structure. Last Spring, J&#038;J&#8217;s McNeil Consumer Healthcare CEO William Weldon<br />
said they would be simplifying the packaging to &#8220;help a mom, dad or caregiver ensure the correct dosing.&#8221; Weldon told The Associated Press then that he thought the new design would become the industry standard.</p>
<p>McNeil spokeswoman Bonnie Jacobs said it was too soon to say when the product will return to the market.</p>
<p>&#8220;We are looking at various alternatives for the redesign of the dosing system and will set a timeline &#8230; once we&#8217;ve reviewed all the options,&#8221; she said.</p>
<p>The company said customers can continue to use the infant Tylenol if the bottle&#8217;s flow restrictor remains intact. If not, they can request a refund by contacting McNeil at 1-888-222-6036 or <a href="http://www.tylenol.com">www.tylenol.com</a>.</p>

<p><b>Hazard: </b>The front wheel can break at the axle hub, causing the stroller to tip and posing a fall hazard.</p>
<p><b>Incidents/Injuries: </b>There have been 36 incidents of the front wheel cracking, including two reports of the stroller tipping over resulting in minor injuries.</p>
<p><b>Description: </b>The recalled Bumbleride Indie strollers are model numbers I-107, I-110 and I-205 with a DOM (Date of Manufacture) from January 2009 through August 2011 sold in multiple colors.  The DOM (Date of Manufacture) can be found on a white rectangular sticker affixed to the side of the seat frame. The recalled Bumbleride Indie Twin strollers are model numbers IT-108, IT-111, and IT-305 with a DOM (Date of Manufacture) from January 2009 through August 2011 sold in multiple colors. The DOM (Date of Manufacture) can be found on a white rectangular sticker affixed to the underside of the handle.</p>
<p><b>Sold at: </b>Buy Buy Baby and other baby product stores nationwide, online at Bumbleride.com, and other online retailers between January 2009 and January 2012 for between $500 and $700.</p>
<p><b>Manufactured in: </b>Taiwan</p>
<p><b>Remedy: </b>Consumers should immediately stop using the recalled strollers and contact Bumbleride to receive a free front wheel retrofit kit.</p>

<p>Pfizer Inc. is recalling 1 million birth control packets after a packaging error has left some with too many or too few &#8220;active&#8221; tablets. Oral birth control is supposed to have 21 active tablets, and 7 inactive sugar tablets.</p>
<p>The affected birth control brands are Lo/Ovral®-28 and Norgestrel/Ethinyl Estradiol Tablets</p>
<p>A company spokeswoman said the problem was caused by both mechanical and visual inspection failures of the birth control. She said the problem has been corrected. Patients with the affected lot numbers should call their doctor immediately and return the birth control packets to the pharmacy. The company urges affected consumers to &#8220;begin using a non-hormonal form of contraception immediately.&#8221;</p>
<p>While the birth control recall does not involve a safety issue, it does pose a serious problem because of unintentional pregnancy.</p>
<p>The affected birth control packets have expiration dates ranging between July 31, 2013, and March 31, 2014.  <a href="http://www.fda.gov/Safety/Recalls/ucm289770.htm" target="blank">Click here to find out specific lot numbers and read the full recall on the FDA website.</a></p>
